Our flexible approach to urgent care at home means that there is always a way of getting your loved one home. It may mean a detailed assessment conversation over the telephone or a local care manager may be close by and able to attend your family home or hospital immediately. We offer an emergency response service to support patients with a terminal illness during times of crisis. We understand that in a crisis situation you need us to react quickly and you may need a care package to start immediately.
In such circumstances we will carry out an emergency assessment in order to facilitate a faster response, so that you are able to return home as quickly as possible. We will work in partnership with other healthcare professionals such as GPs and hospital Doctors, to ensure that all of your care needs are met.
