Who we work with

We work with a range of professionals across health and social care. Our priority is always to make the referral process as simple as possible and to communicate clearly throughout.
Hospital discharge coordinators
We respond quickly to discharge referrals, often confirming care packages the same day the referral is received. We work with families and ward teams to make sure the transition home is as smooth as possible.
Social workers and care managers
We are experienced with local authority funded placements as well as private arrangements. We complete documentation promptly and communicate any changes in a person's needs without delay.
Community nurses and GPs
Where care overlaps with nursing or clinical needs, we coordinate carefully with community health teams to avoid duplication and make sure the care plan is coherent.
Occupational therapists
We implement OT recommendations within care plans and flag any concerns about the home environment or the person's ability to use equipment safely.
