Every specialist condition requires more than a willing pair of hands. It requires knowledge, patience and a consistent presence. Our team is trained and supported to provide all three.

Dementia and Alzheimer's

Familiar faces and consistent routines are the foundation of good dementia care. We work hard to build these from the start, and we do not rotate carers unnecessarily. Our team is trained in dementia awareness and person-centred communication.

Parkinson's Disease

Parkinson's can affect movement, speech, swallowing and mood. Our carers understand the way the condition changes throughout the day and are trained to adapt their approach accordingly, while always maintaining the person's independence where possible.

Post-hospital discharge

Coming home from hospital is a vulnerable time. We can often begin care the day of discharge, supporting recovery, medication management and gradual reablement so that the person can regain as much independence as possible.

Physical disabilities

Whether the disability is lifelong or the result of illness or injury, we support with personal care, mobility, equipment use and maintaining the daily routines that matter to the individual.

Stroke recovery

We work alongside NHS therapists and community health teams to support people recovering from stroke, helping to build confidence, maintain any rehabilitation programme and ensure the home environment is as safe as possible.

Complex health needs

For people with multiple or complex conditions, we coordinate carefully with healthcare professionals and review care plans frequently. Clear communication between care teams and families is something we take seriously.

Our approach

Person-centred, not task-centred.

In specialist care, the risk is that the condition becomes the focus rather than the person. We guard against that. The care plan is always written around the individual, their preferences, their history and what matters to them, not just their diagnosis.

We work closely with GPs, community nurses, occupational therapists and hospital teams where needed. Clear documentation and prompt communication are part of how we operate.

Every specialist care placement is reviewed more frequently than standard visits, and Zoe Rafferty, our Registered Manager, is directly involved in any placement where clinical complexity requires it.
